Care Gap Analytics
Designed an analytics tool that helps providers identify and close care gaps. Complex data made actionable through thoughtful information architecture.
Overview
Care gaps — missed screenings, overdue follow-ups, unfilled prescriptions — cost the healthcare system billions and put patients at risk. I designed a tool that surfaces these gaps to providers at the right moment, in the right context.
The Challenge
Providers were drowning in data but starving for insights. Existing reports were PDF-based, generated monthly, and required cross-referencing multiple systems. By the time a care gap was identified, the window to act had often passed.
Research
Shadowed 8 clinical staff members across 3 facilities. Mapped their daily workflows and identified intervention points where care gap data would be most actionable.
Key insight: providers don’t need more dashboards — they need the right data surfaced within their existing workflow.
Design Approach
Contextual Alerts
Instead of a separate analytics dashboard, care gap alerts appear within the patient chart during appointments. The provider sees relevant gaps at the moment they can act on them.
Risk Stratification
Not all care gaps are equal. Built a visual risk scoring system that helps providers prioritize: which patients need attention first?
Batch Actions
For population health managers, designed a batch view that allows filtering, sorting, and bulk outreach for patient cohorts with similar care gaps.
Results
- Care gap closure rate improved by 28%
